Eastland Memorial Skilled Nursing
Skilled Nursing
Offered through a CMS Swing Bed program, EMH is able to provide a step down program after acute care. Swing Bed is a term Medicare uses to describe a hospital room that can switch from acute status to skilled status. The actual bed and room do not need to change, what changes is the level of care the patient in that room receives. The program is an alternative to prolonged acute care hospitalization and allows qualified Medicare patients who are not ready to return home to continue progress toward self-care. Medicare will cover up to 20 days at 100% coverage providing the patient continues to make progress. This care level is not intended to be a permanent placement and patients return home or transfer to another facility for longer term care.Qualifications
